Prehabilitation phase

We’ll guide you in activating the correct pelvic floor muscles to support continence post-surgery. By targeting these muscles effectively pre-op, we can establish the essential motor planning and sequencing that will serve as a strong foundation for your recovery and further development post-op.

Jack will provide a detailed programme for this stage, including cues to help you activate the right pelvic floor muscles (focusing on the anterior rather than posterior). He’ll also guide you on target activity levels and pad weight monitoring – a proven scientific method to help you reduce reliance on incontinence pads as quickly as possible.

STEP 1

Build pelvic floor strength and speed

In this phase of rehabilitation, once you’ve identified the correct muscles with the prehabilitation programme, we’ll focus on improving both the strength and speed of these muscles. This is essential because the urinary sphincter needs to be strong to handle the demands placed upon it, as well as quick to respond to involuntary actions such as coughing or sneezing.

As up to 90% of a diagnosis can come from history-taking, Jack will carefully listen to your symptoms to ensure you progress appropriately and receive expert guidance to manage any challenges along the way. Jack will also support you through pad weaning, using tried-and-tested techniques to help you regain confidence and resume daily life comfortably.

This phase typically lasts from around 4 weeks to 2 months post-op, depending on individual progress.

STEP 2

Functional Strength

Once you’ve built sufficient strength and speed in your pelvic floor muscles, we’ll progress to more functional exercises tailored to your personal needs and activities. In this phase, we aim to make pelvic floor contractions more automatic by integrating them into functional movements, such as squats, deadlifts, and rotational exercises.

The goal is for these muscles to engage naturally during activities, helping to compensate for the loss of strength in the internal autonomic sphincter. This phase can vary widely in duration, typically starting around 6 weeks post-op and potentially continuing up to 2 years, depending on individual factors that influence healing and muscle strength.

STEP 3

Video Consultations

Video consultations offer a convenient and comfortable way to access men's health physiotherapy, allowing you to receive expert care right from your home. Especially in the period following surgery, when incontinence may be a concern, staying at home can make all the difference. Through video consultations, I can guide you step-by-step through personalised exercises and a bespoke programmes without the discomfort of traveling to a clinic. Is Men’s Health Physiotherapy successful if done remotely?

Men's Health Physiotherapy is well-suited for remote delivery through video consultations, and several factors, supported by research, suggest that this approach can be highly effective. 

Remote consultations offer a convenient and accessible way for men to receive expert physiotherapy care without the need to travel, making it easier to fit appointments into busy schedules or receive care from anywhere. Studies have shown that remote physiotherapy can lead to high levels of patient satisfaction and adherence to treatment plans, thanks to the flexibility and comfort of receiving guidance from home. This approach is particularly effective for conditions like pelvic floor dysfunction and post-operative rehabilitation, where virtual programs have demonstrated similar outcomes to in-person sessions, providing relief and improvement without the need for clinic visits. 

Remote consultations also ensure privacy and comfort, which can be vital when discussing sensitive issues such as erectile dysfunction, incontinence, or pelvic pain. They can reduce the stigma associated with seeking care for these conditions, encouraging more men to seek and continue treatment. Additionally, remote care can be more cost-effective, minimising travel expenses and clinic overheads, while still delivering the proven benefits seen in other health fields, like chronic disease management. FAQs

  • This will be individual; however, we recommend approximately 2-3 sessions pre-op to begin and work on the pre-habilitation exercises. We may see you weekly in this phase, to ensure you are working on the right muscles, and have a true sense of the programme prior to the operation. 

    During your post-operative rehabilitation, we begin with comprehensive strategies such as pad weight monitoring, a proven method to help you gradually reduce your reliance on pads in the most scientific way possible. As you progress to stage two, we will focus on strengthening and increasing the speed of your pelvic floor muscles. The number of sessions required will vary depending on factors like surgical technique, pre-surgery function, and your body's natural healing process, typically ranging from 2 to 10 sessions held fortnightly to allow time for implementing and observing changes. 

    Once you have regained continence and confidence in your recovery, we will move to phase three, incorporating pelvic floor strengthening with functional movements to help train your brain to activate these muscles during daily activities like sitting, standing, or squatting. This helps make these movements automatic, marking the final stage of recovery, where your pelvic floor muscles engage naturally in everyday tasks, ensuring long-term continence. Jack will expertly guide you on when it is safe to resume your regular exercise routine, providing personalized advice and support to help you return to your training program with confidence. 

  • In-between sessions you will be working on your individualised training programme, bespoke to the phase of rehabilitation you are in.  The level of intensity of the exercises will vary daily, however you will be required to work on the programme daily to see optimal results.  You can reach out to Jack via email at any time throughout your recovery if you have any questions regarding the programmes.

  • No equipment will be required to within the first two stages, it will be all done through advice and guidance and through ‘coaching cues’ to help you activate the right pelvic floor muscles. When we progress to stage three, Jack will recommend certain exercises for you to do that may require either a dumb-bell, kettlebell or resistance band.

  • You will have the urinary catheter in for 10 days post-op, therefore you will not want to do the pelvic floor exercises as it will be too uncomfortable. So, we say to resume the exercises once the catheter is out. Please re-start the phase 1 exercise programme, and then on our first appointment post-op we will go through the next phase of the programme.

  • There are a lot of factors at play that will determine your recovery post-prostatectomy. This includes surgical technique and skill level of the surgeon, the base-line level of pelvic floor and core musculature prior to the operation, as well as your own body's ability to heal. As everybody is individual, recovery post-prostatectomy can vary from anywhere between 8 weeks – 6 months. There are precautions in place for six weeks, such as no heavy lifting over 5kg, this is due to risk of incisional hernias. Additionally, it takes six weeks for soft tissue healing to take place, therefore it is likely you will be incontinent for the first couple of weeks after the operation, which is down to inflammation and weakness of the autonomic internal sphincter muscles that have been severed and stitched back to the bladder to compensate for the prostate removal. We therefore recommend you take it easy for the first six weeks, and then we are there to help you build back up supersede your baseline level of fitness and help you reach your goals again!
